I, the above listed parent/guardian, give permission to First Baptist New Braunfels and its staff/adult volunteers to transport my child/children to and from Pineywoods Baptist Camp in Woodlake, Texas.

In the event that an emergency occurs during the FBNB Pineywoods Camp event and I, the parent/guardian, cannot be reached, I authorize the responsible adults (Student Pastor, adult volunteers, or host home parents) to seek and obtain emergency medical care for my child. I, the parent/guardian, give my consent for necessary emergency medical treatment. I understand that it is my responsibility as the parent/guardian to provide FBNB Student Ministries with any changes in medical/insurance/emergency/contact information for my child.
